An initial consultation usually takes up to 1 hour.
This time allows for:
Thorough discussion of your medical and lifestyle history.
Dietary assessment and evaluation.
Provision of advice tailored to your specific requirements (This professional plan is subject to the normal provisions of copyright).

Also, we don’t charge extra for some email support* – So if our patients have a burning question between visits, we would love to answer it.
* Of course if you have a looong list of Qs, it would be better to discuss them in a consultation.
Patient consultation fees
Initial Standard Consultation: $220 (up to 60min).
Review Consultation: $95
Review Consultation Extended: $150
If you have Health Fund “Extras” cover, a rebate may be claimed off fees as Mark has the requisite Provider Number.
Fees under a Medicare Program
Mark is a registered professional Dietitian with the Government’s Enhanced Primary Care (EPC) Program under Medicare.
You do not pay any fees for follow up visits to Mark under this program. NOTE- You may be charged a gap only for the initial visit. You can have up to 5 visits to Mark as a Dietitian per calendar year.
Or 10 visits under a GP Eating Disorders Plan (EDP).
Therefore we suggest you first check whether your GP is a participating Doctor. If “Yes”, book an appointment emphasising requirement is for a Primary Care Plan. If your GP agrees, the relevant Referral Forms (GP Management Plan) would be prepared by the GP, to Mr Mark Surdut, 139 Hawthorn Road, North Caulfield – for 5 services. Please have the Plan EMAILED to firstname.lastname@example.org.
Mark then handles all paperwork with Medicare on your behalf.
Changing an appointment date
Industry practice requires 24 hours notice of an appointment cancellation please. Otherwise, depending on circumstances, the normal applicable fee may apply.
Fees for other services such as Workshops and Writing can be discussed.
